Third Party Consent Disclosure

By clicking, “Accept,” I/We authorize Premia Mortgage, LLC dba Premia Relocation Mortgage, to obtain from third parties, including but not limited to those third parties listed below, any and all verification information and documentation that Premia Mortgage, LLC dba Premia Relocation Mortgage may request as required to process my/our mortgage loan application. Such information shall include, but is not limited to, employment history and income or wages; bank, money market, and similar account balances; verification of assets; credit history; copies of income tax returns; and rental/mortgage verifications. By authorizing Premia Mortgage, LLC dba Premia Relocation Mortgage, I understand l also agree to the Third Party’s applicable Privacy Policy, Terms and Conditions and other similar agreements listed below (as updated from time to time by the Third Party) as a condition to Premia Mortgage, LLC dba Premia Relocation Mortgage receiving information or verification from such Third Party. This authorization is valid for any party named in the preapproval or loan application. I/We understand and agree that Premia Mortgage, LLC dba Premia Relocation Mortgage reserves the right to change the mortgage loan review process to a full documentation program. This may include verifying the information provided on the application directly with the employer and/or the financial institution.
